MANDATORY FORMS (click on the name of the form to download a copy):
BALTIMORE COUNTY WAIVER FORMS: This form must be signed be each and every parent before their child
participates in your program. A new form is required for program they participate in.
FUND-RAISING FORM: Before you commence with any fund-raisers, you MUST complete this form and turn it into the ESP
Executive Board Fund-Raising Chairperson for approval.
INCIDENT REPORT: This form MUST be filled out if their is a head injury or injury that requires medical attention. Turn
them into Denise Spurrier in the ESP Rec-Office at Edgemere ES.
ESP PROGRAM CHAIRPERSON HANDBOOK: Email Tim Dodge (tcdodge@yahoo.com) for a copy. This handbook is
designed to offer each program ideas and guidelines to manage a healthy and successful program. It also includes
important contact information.
SPONSORSHIP FORMS: This will take you to the 'One-Community' Sponsorship page so that you may download the
resources you need to solicit financial support for your program. ALL sponsorships must be submitted to the ESP Executive
Board Sponsorship Chairperson.

the first night you request) and flyers to be sent out to local schools.

HEADS-UP CONCUSSION TRAINING: Please take the following steps to ensure you are certified to coach with the ESP
Rec-Council.
ATTENTION ALL ESP COACHES (Head & Assistant)
1. Click on the 'HEADS-UP' icon to the left to take the FREE online concussion training course.
2. Watch the 25 minute video training course.
3. Type in your name on the PDF Concussion Training Certificate and save as:
"Your Name" HeadsupCertificate
4. Email the PDF file to your ESP Program Chairperson
5. Your training course certification is good for all ESP sports for one (1) year from the completion date.
